General Functions:

Member of the interdisciplinary team providing case management and clinical services to persons with mental illness, who may also have a secondary diagnosis of ID/DD. Must be accessible via cellphone during work hours and share monthly on-call responsibilities.

Key Responsibilities

The Mental Health Support Specialist is responsible for assessing the need for and overseeing the coordination of services for caseloads through clinical assessment, benefits maintenance, coordination of health services, and community support.  The Support Specialist works to build relationships with entities to ensure successful care coordination.

Minimum Qualifications:

Master’s degree in Social Work or related Human Services field with at least three years’ experience in social services. Must have excellent written and verbal communication skills.  Sufficient decision making and problem solving, computer and organizational skills. Individual must be able to work autonomously.  Must have valid driver's license and approval through agency's insurance carrier to drive agency's vehicles.  Must pass Criminal Background Check and Nurse’s Aide Registry. 

Hours: Monday – Friday 9am-5pm with flexibility
