Minimum Qualifications:  MA/MS in social work, psychology or related field preferred and at least 1 year of paid full-time experience working with adults with developmental disabilities or BA/BS degree in social work, psychology, or related field plus a minimum of 3 years paid full-time experience and at least one year working with adults with developmental disabilities. Experience working with individuals with mental illnesses also preferred. Must have the ability to complete all required documentation. Excellent organizational skills. Good computer skills and ability to utilize Microsoft Office.  Able to lift up to 30 pounds. Qualified candidate must possess a valid and clean driver’s license, proof of vehicle insurance and must have their own vehicle. Must pass Test of Adult Basic Education (TABE), Criminal background check, Nurse’s Aide Registry and CANT’s background check.

General functions: The Case Manager is responsible for the completion of all necessary assessments. This position also holds responsibility for preparing, conducting and completing the 6-month and annual IPP as well as any necessary amendments. The Case Manager also works closely with the Clinical Supervisor and DSPs to properly implement the IPP. Responsible for ensuring monthly data, other documentation, and MRO billing is completed.  Responsible for maintaining the quality of services provided to the members.  Responsible for collaborating with the Clinical Supervisor for all programming, services and needs of the members. On-call duties are required with this position.

Hours: Full-time-Salaried employee. Must have flexibility with schedule and the ability to work two weekend shifts per month and some holidays. Preferred work hours will generally be between the hours of 7:00 a.m. and 7:00 p.m. The position typically works 40 hours per week but may vary based on program needs or as assigned by the Clinical Director.
