Envision Unlimited is excited to bring aboard a Heath Services Coordinator to oversee our the coordinated care of our members. 

General Functions:

Using available resources, the Health Services Coordinator promotes and restores health by developing day-to-day management and long-term planning for the client’s care.  This is done through hiring, directing, and developing staff and collaborating with doctors and multidisciplinary professionals to provide physical and psychological support for clients, guardians, and parents.

Qualifications:

Current license from the State of Illinois as a Registered Nurse, w/ Nurse Training Certification 

CPR certified

Be at least 21 years old

Working with the Developmental Disability population for at least 5 year.

Able to pass a criminal background check

Excellent written and oral communication skills

Valid Il driver’s license

10 year of supervisor experience
