Description

Purpose:  
The basic functions of the Unit Coordinator are to develop, implement, and manage client cases through staff personnel to ensure that client contracted training programs are functioning effectively, consistent with the mission of the agency.

Qualifications:

  • Bachelor’s degree from accredited college or university in Rehabilitation Counseling, Psychology, Social Work, Special Education, or related social service field, with a minimum of one years experience in developmental disabilities along with supervisory experience .
  • Must be able to meet qualifications of QIDP. Must be able to complete the requirements for placement on the State of Illinois Direct Support Persons Registry.
  •  Pass a full background check.
 Essential Responsibilities:
  • Coordinate, supervise, and monitor staff and client activities to ensure that training programs and activities are correctly implemented.
  • Design, develop, and prepare initial, semi-annual, and annual client program plans; analyze individual skills and abilities, and establish goals and methods to achieve planned objectives.
  • Develop, schedule, and implement a variety of client staffings; initiate and conduct staffing meetings with I.D. Team; analyze client activity records, review program plan.
  • Ensure weekly / monthly billing and reporting of client attendance and Title XX activities are completed accurately and on time.
  • Maintain current client habilitation files in accordance with case file organization and retention schedule.
  • Participate in staff training as assigned and maintain continuing education as required.
  • Ensure that client programs and activities are in compliance with funding source requirements and arrange special staffings with funding source representatives.
  • Act as liaison between unit rehabilitation and production departments to resolve staff or client problems related to meeting work schedules and quality standards.
  • Respond to medical and behavioral emergencies following appropriate health & safety and crisis prevention procedures. Monitor staff members performance on following established procedures for responding to emergencies.
  • Monitor staff activities to ensure that required documentation is maintained, staff plans are implemented, and supplies are available as needed and that client personal care, hygiene and medical needs are met.

 

Location: North and Southside of Chicago
Full-time position- Mon- Fri 8:30a m-4:00 pm
