Care Coordinator - Community Transition Program

General Functions: Provide counseling and care coordination to clients transitioning from State Operated Mental Health Facilities (SOF) into the community.

Key Responsibilities:

Care Coordination:

• Provides direct service to clients with severe and persistent mental illness. 
• Provide linkages to primary and specialty care including psychiatry, including Envision Unlimited’s mental health programs.
• Assist with transition activities such as linkage and referrals for social service supports, medication transfers and prescription refills and transportation when clients exit the facility.
• Work with provider agencies to ensure client attendance at medical appointments and compliance with physician’s orders.
• Attend client staffings as part of the multi-disciplinary team

Counseling & related services:

• Develop treatment plans based on client’s assessed need
• Provide brief, supportive, goal directed counseling to clients
• Provide therapy to clients with trauma histories, severe and persistent mental illness as needed
• Work with team to develop therapeutic and skill building group services

Minimum Qualifications:

Position requires a master’s degree in social work, psychology, counseling or related field. Must have experience working within the mental health field and a minimum of 3 years’ experience working with complex clients. Knowledge of Medicaid standards and IM/CANS certification a plus.
