Description:

Our Dual Project is established to provide mental health services to people with both a mental health diagnosis and an intellectual or developmental challenge. Comprehensive services are provided by a multidisciplinary team under the direction of an LPHA, and include behavioral analysts, counselors, and case managers. The following rehabilitative services include: 1) Assessment, 2) individual and group therapy 3) All Mental Health services funded by Medicaid that an QMHP-level staff member can provide, 4) crisis services, 5) medication services.

General Functions:

The Clinician will serve as a member of a multi-disciplinary team providing comprehensive mental health services to clients residing in Envision’s Residential Program or engaged in services at one of our Community Day Treatment Facilities. Clinician will be responsible for completing a comprehensive mental health assessment which also include LOCUS, SLOF and other tools to arrive at a mental health diagnosis. A treatment plan will also be completed based on assessed needs. The clinician will be providing individual and group therapy, medication training, skill building, crisis intervention and case management services with clients using a recovery model in a home/community-based model.

Key Responsibilities:

  • Work as a member of a multi-disciplinary team to implement recovery-oriented services to clients using evidenced based practices.
  • Work collaboratively with team leader and staff attending case staffing and clinical meetings as needed. 
  • Engage clients in assessment and treatment planning process.
  • Provide services needed to achieve psychiatric stability including crisis intervention, therapy, and medication training.
  • Provide training, consultation, technical assistance and support to residential staff in key areas such as MH diagnosis, trauma informed care, resiliency and recovery.  
  • Work with residential staff to facilitate individual and group skill building activities to aid clients to achieve recovery goals and promote community living at highest potential possible
  • Enter progress notes in Envision’s Electronic Health Record System (Quantum).
  • Complete required productivity targets and documentation on a timely basis
  • Work with leadership team to manage client recruitment, prior authorization and on-going utilization management and other administrative requirements of the program

Minimum Qualifications:

Master’s Degree in social work, counseling, psychology, or related field. Experience working with clients who have a intellectual or developmental disability a plus. Experience in working directly with clients with severe mental or physical illness also preferred. Must be able to work with diverse populations with multi-faceted needs. Excellent written and oral communication skills needed, with demonstrated ability to engage clients. Valid driver’s license, with insured automobile to travel to multiple locations required. Knowledge of Medicaid rule 132/140 a plus.
