General Functions:

Case Aide will work directly with clients who are in the process of transitioning or who have transitioned from nursing homes and long-term care facilities into their own apartments within the community. Case Aide will participate as a member of the multidisciplinary team and provide support to staff responsible for the supervision, training, and implementation of an individual’s care plan. Facilitate all client activities, and complete documentation as required by the multidisciplinary team, CARF Accreditation and funders including Medicaid standards.

Key Responsibilities:

  • Secure vital documents (SS Cards, Birth Certificates) needed for transitions
  • Assist class members to secure State IDs at the Secretary of State’s Office
  • Secure Medical Records from Nursing Homes and Mental Health Rehab. Facilities
  • Facilitate pre-transition activities such as arranging transportation, shopping for furniture, groceries and needed supplies.
  • Arranging and providing transportation to medical and psychiatric appointments
  • Provide clients with training, technical assistance and support needed to maintain successfully in the community.
  • Provide compassionate care and recovery-oriented services to clients with severe and persistent mental illness.
  • Provide input into the client’s care plan by outlining strengths, challenges, and accomplishments.
  • Assist clients to achieve independent living goals by providing training in key areas such as activities of daily living, budgeting, transportation, developing healthy relationships and other items defined by ITP goals.
  • Develop and implement psychosocial recreational opportunities both on site and off site.
  • Document all activities provided in an electronic health records system
  • Achieve and maintain productivity targets reflective of case load size and client needs.

Minimum Qualifications:

  • Be at least 21 years old
  • High School Diploma or GED (Bachelor’s Degree Preferred)
  • Able to pass a criminal background check
  • Excellent written and oral communication skills
  • Flexibility in Schedule
  • Willing to engage in a variety of tasks and activities
  • Valid IL driver’s license
  • 1 year of social service experience, preferred
