General Functions:
Provides outreach and engagement services to clients who currently reside in State Operated Mental Health facilities (SOF). Assist with case management activities for clients transitioning to the community.
Key Responsibilities:
Provide outreach and engagement services to individual clients residing at in-patient facilities
Describe various transition options available & distribute literature on Envisions Services
Share stories of own success in accordance with comfort level
Facilitate group conversation with clients, family members and legal guardians
Accompany clients on initial appointments, recreational activities and PSR Center events
Minimum Qualifications:
High School Diploma or GED. Applicants must have lived experience with recovery from mental health, medical issues or addiction together with working with larger systems such as State Departments of Mental Health or Aging. Experience with transitions from in-patient to community settings preferred. Knowledge of Medicaid is a plus. Must feel comfortable with public speaking and have excellent oral and written communication skills. Must be able to drive or utilize public transportation to multiple locations. Preference given to Certified Recovery Support Specialists.
