General Functions: Provide care coordination to clients transitioning from nursing homes or long-term care facilities into the community.

 

Key Responsibilities:

Assist with transition activities such as securing durable medical equipment, addressing accessibility issues, medication transfers and prescription refills.

Arrange for home and community-based waiver services for eligible clients.

Provide input in clients service plan

Provide linkages to primary and specialty care including psychiatry

Provide oversite to client’s healthcare needs such as annual physicals, lab work, dental and eye care.

Work with care managers to ensure client attendance at medical appointments and compliance with physician’s orders

Secure medical transportation services, as needed

Maintain medical documentation in the electronic health record system

Attend client staffings as part of the multi-disciplinary team.

Minimum Qualifications:

Position requires a master’s degree in social work, psychology, counseling or related field. Must have experience working within the mental health field. A minimum of 3 years’ experience working with complex clients is also needed. Medical case management experience a plus. Knowledge of Medicaid standards is a plus. LCPC, LCSW, LMFT, RN are preferred.
