Description

Qualifications: High school diploma; Bachelor’s degree preferred. Minimum of one year work experience in rehabilitation, social work, special education or related field. Knowledge of business and industry practices helpful. Must have a valid driver’s license and the ability to travel per the needs of the agency.


Purpose: Perform functions to prepare, support, and train clients for employment in the community.

 

Essential Responsibilities:

  • Train clients to complete work assignments per employer specifications.
  • Monitor and report client progress based on established goals and objectives in a timely and efficient manner.
  • Assist employers in communicating with the client to facilitate client/employer relations.
  • Complete daily time sheets, case notes, and reports for all clients in your caseload and route these documents as necessary.
  • Communicate service and active treatment needs of clients to the Job Coach Supervisor.
  • Participate in client staffing.
  • Conduct job trial evaluations for new clients.
  • Perform job development activities that offer current site expansion or the development of new sites.
  • Responsible for agency vehicle operation and transporting work crews.
  • Ensure safe working environment and practices for clients at work sites, including review of fire and emergency procedures to assure prompt and safe responses.
  • Write and file client progress notes and Title XX documentation on proper forms.
  • Participate in regular staffing reviews and follow through with recommendations.
  • Respond to medical and behavioral emergencies following appropriate health & safety, crisis prevention, first aid, CPR, accident/incident reporting and other procedures, as necessary.
  • Attend to personal care (such as toileting, showering, tooth brushing), hygiene (such as hand washing) and medical needs of clients.
  • Attend mandated training within prescribed time periods and attend other in-services, as requested.
  • Support clients in self-advocacy activities.

Other Responsibilities:

  • Attend regularly scheduled staff meetings.
  • Participate on agency committees.
  • Distribute client checks when appropriate.
  • Assist in client placement at job sites.
  • Perform other related duties as assigned.
