Description:
The ACT Program is the integration of crisis intervention; treatment services and rehabilitative supports focused on skill building and stabilization to promote and maintain community living. ACT services are available to the client 24/7 are provided by a multidisciplinary team under the direction of an LPHA, and include the following rehabilitative services: 1) All Mental Health services funded by Medicaid that an MHP-level staff member can provide, 2) crisis services, 3) medication services, and 4) individual and group therapy 5) all Case Management services.
Minimum Qualifications:
- Clinical Licensure (LCSW, LCPC or LMFT)
- Master’s Degree in social work, psychology, counseling or related field
- Minimum of 3+ years working with clients diagnoses with mental illness
- Familiarity with Medicaid, rule 140 standards and substance abuse services a plus
General Functions:
Licensed Team Supervisor will be responsible for providing clinical supervision, technical assistance and support to direct service staff working with clients diagnosed with serious mental illness. Supervisor will provide clinical direction of the entire process for clients from admission to discharge. Responsibilities include providing oversight of the evaluation process, treatment planning, resource development and any other task needed to ensure adequate supports are in place so clients will remain safely and successfully in the community. The supervisor will interface with other programs to identify clients in need of ACT services. Many will be transitioning from nursing facilities, psychiatric hospitals and specialized mental health rehabilitation facilities and the supervisor will work with staff on the transition process. The supervisor will coordinate with other leadership staff to ensure completion of quality improvement activities, data collection, outcome measures, management, monthly reporting, financial tracking and budgeting.
Key Responsibilities:
- Responsible for participating in decision making on an administrative level in conjunction with the Director.
- Responsible for assisting with the development of policies and procedures, program planning, implementation and evaluation
- Supervisor will facilitate interviewing, hiring, and coordinating, and/or directly training, new staff members.
- Supervisor will carry out progressive disciplinary action with staff members as needed.
- Responsible for designing individual and group training opportunities for staff based on needs of program and identified development needs for staff.
- Responsible for maintaining Key Performance Indicators as specified by the funder and completing quarterly funder reports
- Responsible for proving direct supervision and training to Direct service staff. Documentation on coaching logs required for each staff member
- Ensure all services provided are in accordance with client’s individual treatment plan.
- Will ensure the completion and compliance maintenance for staff in all clinical documentation related to span of control as outlined in the fidelity model.
- Ensure assigned staff complete required productivity targets and documentation on a timely basis, including assessments, treatment plans and other clinical documentation.
- Will work with Director to recruit clients, ensure authorizations are in place and clients receive adequate services to support optimal health and wellness.
- Participates in a rotational after hours on call system, with additional compensation
- Work with leadership team to manage prior authorization and on-going utilization management of program
