General Functions:

Psychiatric Nurse is a member of the administrative team, responsible for overseeing and insuring efficient day-to-day operations of the clinic, and nursing related care for members with mental illness.  The MH Nurse is directly responsible to, and under the immediate supervision of the Vice President of Mental Health Services and is expected to collaborate with, and work alongside, the Director of Mental Health services and the mental health supervisory team.

Key Responsibilities:

  • Responsible for completing annual evaluations for clients w/medically complex needs. This includes record review, client interviews and collateral contacts as needed.
  • Responsible for monitoring and scheduling client medical appointments, ensuring that annual exams are tracked, and in compliance with State regulations at all times.
  • Responsible for providing injectable psychotropic medication administration under the direction of the psychiatrist
  • Responsible for reviewing, assessing, and reporting member’s lab results to the psychiatrist and/or medical doctor.
  • Responsible for taking vital signs for the doctor and preparing the Physician Order Sheet and any labs as needed.
  • Serve as a liaison for pharmacies and internal/external medical professionals regarding clients’s clinical and medical care.
  • Responsible for communicating any medication needs, and faxing all orders, to the pharmacy.
  • Ensures prior authorization for medications is obtained and/or assist with appeals.
  • Will participate as member of a multi-disciplinary team that will engage in client service planning, implementation and evaluation
  • Nurse will provide recommendations on living arrangement, needed resources and supports.
  • Nurse will work with program staff to secure assistive and medical devices and any other resources within span of control
  • Nurse will provide training to employees, care givers and family, as needed
  • Responsible for providing training, technical assistance, support and consultation to program staff.
  • Responsible for responding to any medical emergency that may arise and completing incident reports as necessary.
  • Will conduct in home visits for clients when clients are discharged from the hospital and/or in frequencies needed to ensure clients are safe and well cared for.
  • Responsible for completing all Medicaid Rule 132/140 service documentation requirements and achieving productivity targets as assigned.

Minimum Qualifications:

  • Must have a current state of Illinois RN License.
  • Must have at least two years of related experience working with individuals with serious and persistent mental illnesses within the past five years.
  • Able to complete mandatory annual trainings.
  • Must have a valid driver’s license.
  • Must have the ability to walk, bend, stoop, sit, stand, reach, and lift 50 pounds
  • Familiarity with electronic health records and Medicaid standards a plus
