MINIMUM QUALIFICATIONS: Regional Program Supervisor must have a master’s degree in social work, counseling, psychology or related field, and be clinically licensed (LCSW/LCPC/LMFT) with 3+ years’ experience in the social service field, with at least 1 year in a mental health program. Must have minimum 1 year supervisory/managerial experience and must have strong oral/ and written communication skills.

GENERAL FUNCTIONS: Regional Program Supervisor is a member of administrative team responsible for directing recovery-oriented treatment of clients as they transition from nursing homes and long-term care facilities (SMRFS) to a community setting. Responsible for providing oversite of client resident review/assessments, service planning, transition to community living arrangement, on-going monitoring, care coordination, and general wellbeing of clients post transition. Supervisor will provide direct supervision and training to mental health staff responsible for client service delivery. Will work alongside Director to further develop and expand programming.

ESSENTIAL FUNCTIONS:  

  • Responsible for participating in decision making on an administrative level in conjunction with the Resident Reviewers for Williams –Colbert and Program Manager.
  •  Responsible for maintaining compliance with DHS, and court requirements for program
  • Responsible for assisting with further program development, as well as the development of policies and procedures, program planning, implementation and evaluation of the program to ensure quality, recovery-oriented services are provided to all members.
  • Responsible for designing individual and group training opportunities for staff based on needs of program and identified professional development needs for staff.
  • Responsible for maintaining Key Performance Indicators for program services.
  • Responsible for providing direct supervision and training to mental health staff responsible for providing services, while maintaining supervision logs for each staff member, carrying out progressive disciplinary action with assigned staff if/when needed.
  • Responsible for interviewing, hiring, and coordinating, and/or directly training, new staff.
  • Will attend weekly supervisions with Program Managers and meetings as requested, participate in committees and client events as needed, and collaborate on marketing and outreach efforts.

CLINICAL EXPECTATIONS:

  • Will work with staff to ensure completion of resident reviews/assessments occur annually for all CM’s.
  • Will provide oversite of client service plans pre and post transition and ensure that resident reviewers obtain CM signatures for service plans.
  • Ensure all services provided are in accordance with client’s individual service plan.
  • Will provide and document crisis interventions and additional risk assessments, as needed, and will complete incident reports when appropriate within a 24hr period.
  • Enter, maintain, and update client information in Envision Unlimited’s Electronic Health Record (Quantum). As required
  • Will monitor and ensure the timely completion, and ongoing compliance of clinical documentation for staff with primary/caseload responsibilities.
  • Serve as the LPHA reviewing and signing off on documents for staff with primary/caseload responsibilities
  • Assist and monitor to staff to ensure required productivity targets are met within required time frames.
  • Attend all scheduled clinical meetings including group supervisions, clinical staff team meetings and all staff meetings.

OVERALL JOB PERFORMANCE:

  • Demonstrate flexibility and adaptability with changes to program needs and schedule as the need arises.
  • Participate in/attend committee meetings as assigned.
  • Attend special events for clients and assist as needed.
  • Attend internal and/or external trainings when available and approved (minimum of 15 hours/year). Be prepared to present information from outside trainings to clinical staff.
  • Inform Director of Mental Health of any condition that impacts the safety and wellbeing of clients served. Seek assistance as necessary to mitigate risk.
  • Adhere to all the Policies and Procedures of Envision Unlimited.
  • Maintain confidentiality according to the Illinois Mental Health code and Confidentiality Act at all times.
  • Maintain a professional attitude in conduct, and when interacting with other staff, clients, and outside personnel.
  • At all times, maintain a professional image and role as a representative of Envision Unlimited.
  • Demonstrate conduct and behaviors consistent with agency mission and policies.
  • Follow good clinical ethical practices with clients and seek supervision as needed.
  • Respond sensitively to the diverse populations of the agency, including persons with disabilities and/or mental health issues.
  • Arrive to work, meetings, and all work-related functions on time. Report any absence, tardiness, or schedule change to supervisor as soon as possible. Maintain appropriate attendance record using Paylocity.
  • Other duties as assigned
