Envision Unlimited is excited to bring aboard an Outreach and Engagement Specialist to provide care coordination services as part of a diversion program for clients at risk of long-term in-patient care.  

Minimum qualifications: Bachelor’s Degree in social work, counseling, psychology or related field, Master’s preferred. Three years of experience in working within the social service arena, including internships with two years working directly with clients with serious and persistent mental illness. Must be able to work with diverse populations with multi-faceted needs. Excellent written and oral communication skills needed together with demonstrated ability to engage clients.

Position requires a licensed, insured automobile and ability to travel to multiple locations is part of the job.

Hiring preference given to staff with CADC License, IM/CANS certification; experience with crisis programs such as ACT, CST or First Responders programs. Knowledge of Northside providers and social services strongly preferred. 

 Viable candidates will have knowledge of Medicaid Rule 132, a recovery approach to clinical treatment, A candidate efficient at developing, implementing and completing ITPs and overseeing the coordination of care will be most successful.

General Functions:

Outreach and Engagement Specialist will conduct on-site evaluations with clients identified by the Pre-Admissions Screener and Resident Reviewer (PAS-R) who are currently residing in an in-patient setting such as Psychiatric hospitals, State Operated Facilities, Crisis Residential and others. Position provides counseling, linkage case management, assistance with applications for entitlements, crisis plan development and care coordination activities.
