 General Functions:

PSR a Group Facilitator serves as a member of the interdisciplinary team and provides comprehensive services to individuals with mental illness. Some clients have co-occurring cognitive challenges, substance abuse issues and minor medically complex needs. Instructors are afforded an opportunity to teach clients new skills to aid them in achieving their highest potential possible. It is an exciting opportunity to really make a difference in people’s lives.

Group Facilitator must creatively plan recovery-based activities and actively engage the program participants throughout the day. The Group Facilitator will focus on providing group and individual services that will actively engage members in the following areas: independent living skills, community integration, economic self-sufficiency, symptom management, coping skills, peer support and future planning. Participates in the development and implementation of individual treatment planning, curriculum development, therapeutic recreation and all client related activities and documentation as defined by the interdisciplinary team, accreditation and funding organizations. Documentation responsibilities in PSR include the completion monthly summaries and progress notes that are compliant with rule 132/140 (Medicaid) standards.

 

Key Responsibilities

-Provides direct service to clients with severe and persistent mental illness. 

-Develop curriculums for group services that support client’s recovery

-Create, implement and evaluate skill building and therapeutic groups

-Utilize evidenced based practices with clients

-Document services within an electronic health records system

 

Minimum Qualifications:

Master's degree in social work, psychology or related human services field required. One-year paid experience working with mental health clients strongly preferred.

Must be at least 21 years of age, possess a valid driver's license, be approved to drive agency vehicles through agency insurance, and pass a required criminal background check. Must have effective oral and written communication skills, ability to work with diverse populations and work collaboratively with other staff. Knowledge of Medicaid standards is a plus.

Hours: Monday – Friday 8:00am – 4:30pm
