General Functions:

The basic function of the Day Program QIDP (Unit Coordinator) is to ensure appropriate programming (internal and in community) is developed and implemented for individuals served via Direct Support Professionals (DSPs) supervised and via direct case management and personal care planning activities.

Key Responsibilities

  • Develop and implement opportunities for Community-Based Inclusion activities based on the interests of individuals served. Develop and enhance community partnerships.
  • Coordinate, supervise, and monitor staff and client activities to ensure that functional, age-appropriate, meaningful, and engaging activities are developed and appropriately implemented.
  • Participate in person centered planning, providing input for discovery tools, attending Personal Plan meetings, and alongside individuals served, develop implementation strategies.
  • Ensure Implementation strategies are followed and appropriate documentation, including QIDP monthly summaries, are completed in a timely manner.
  • Ensure assessments and documents (i.e., ICAP, Human Rights Restriction, etc.) are completed accurately and in a timely manner.
  • Maintain current client habilitation files in accordance with case file organization and retention schedule.
  • Participate in staff training as assigned and maintain continuing education as required.
  • Liaison effectively with ISCs, guardians, family members, residential providers, Job Placement staff and other stakeholders in order to provide cohesive supports and advocacy for individuals served.
  • Respond to medical and behavioral emergencies following appropriate health & safety, crisis prevention, OIG reporting, and CIRAS reporting procedures are followed. Monitor staff members performance on following established procedures for responding to emergencies.
  • Default hours are Mon- Fri 8:30a m-4:00 pm. Periodic evening and week-end hours required. 

Minimum Qualifications:

Bachelor’s degree from accredited college or university in a human services field (including but not limited to psychology, sociology, special education, social work, or rehabilitation counseling) OR in recreation or in a specialty area such as art, dance, music, or physical therapies.

A minimum of one year experience working with individuals having intellectual/ developmental disabilities

Must be able to meet qualifications of QIDP. Must be able to complete the requirements for placement on the State of Illinois Direct Support Persons Registry. Must be able to become CPR/First Aid Certified. Must be able to pass Safety Care.

Pass a full background check.
